"Your request must be received by our office within 90 calendar days from the date of this notice." Its form, DHS 1161 (Rev. 12/18), lists the actions it is for — "My application was denied." "My redetermination for continued medical assistance was terminated." "My coverage or service was denied or terminated." — and none names a nursing facility's decision. The division prints, for member grievances and appeals, "808-692-8094" and "Department of Human Services Med-QUEST Division Attention: Health Care Services Branch P.O.
